Asthma Education Gift Card

Texas Children’s Health Plan rewards you for taking control of your asthma! When you don’t have any symptoms, you won’t cough or wheeze, you can run and play as you want, you won’t have asthma attacks, and you won’t have to go to the emergency room. When you complete a series of 6 Asthma Educations classes, we’ll send you a $50 gift card.

How do I claim my benefit?
You can register for this class by calling Member Services, or visit the Texas Children’s Health Plan web site.

How will I receive it?
Upon registration, you will receive a confirmation email with the time and location of the class. All classes will be held at The Center for Women and Children, either Greenspoint or Southwest locations.

Who is eligible?
This class is available at no cost to members ages 6 through 18 with and asthma diagnosis, that are on the STAR, CHIP, or STAR Kids programs.

Is this gift available in my service area?
Class is available to members within the Harris Service area.


Post Hospitalization Gift Card

Post Hospitalization Follow-up Gift Card

Research has shown that good planning and a follow-up doctor’s visit can improve your health and reduce the chance of readmission. We encourage your child sees their doctor within 14 days of your child’s release from the hospital. We’ll even send you a gift card!

How do I claim my benefit?
You will receive a Texas Children’s Health Plan post card that you’ll need to take to your physician. At the end of your post hospitalization follow-up visit, ask your physician to complete and sign the card. Then, mail the post card, at no cost to you, to Texas Children’s Health Plan. You’re eligible to receive this benefit up to 3 (three) times per year. You may also request this post card by contacting your Service Care Coordinator directly or by clicking here.

How will I receive it?
You will receive your gift via U.S. Mail.

Who is eligible?
This benefit is available to our members that are specifically on the STAR Kids program.

Is this gift available in my service area?
Benefit is available to members within all service areas, including Harris, Jefferson, and Northeast.


Diabetes Gift Card

Diabetes Gift Card

Take steps to manage your diabetes and get rewarded for improving your health! Receive a $20 gift card, up to $60, when you complete your Annual Diabetic Eye Exam, complete your Biannual HbA1c Blood Test, or Maintain a HbA1c blood result under 8.

How do I claim my benefit?
You will receive a post card that you’ll need to take to your physician. The post card will have three areas, one for exam action item completed. Ask your physician to complete the appropriate portion, stating completion of the item. Then, mail the post card to Texas Children’s Health Plan. Members are eligible to receive this benefit up to 3 (three) times per year for a total of $60 in rewards, $20 per action item completed. You may Click Here to download the post card.

This benefit is available to our members 18 years of age or older and participating in Texas Children’s Health Plan Case Management.

Cervical Cancer Screening Gift Card

Cervical Cancer Screening Gift Card

Every woman age 21 and older should get a cervical cancer screening (pap smear) during her well-woman exam. We’ll send you a $20 gift card for completing yours!

How do I claim my benefit?
You will receive a post card that you’ll need to take to your physician. Ask your physician to complete the post card once the well-woman exam/pap smear, is completed. Then mail the post card, at no cost to the member, to Texas Children’s Health Plan.

How will I receive it?
You will receive your gift via U.S. Mail.

Who is eligible?
This benefit is available to our members that are not pregnant, ages 21 through 64, and are on the Texas STAR program.

Is this gift available in my service area?
This benefit is available to members within Harris and Jefferson service areas.
 


Smoking Cessation Benefit

Smoking Cessation Benefit

Texas Children’s Health Plan will help the parents of members under 20 years of age with up to $75 towards smoking cessation products.

How do I claim my benefit?
Parents of Texas Children’s Health Plan members looking to claim this benefit will need to contact their Service Care Coordinator, Case Manager, or have received a physician’s referral. You also may contact Member Services.

How will I receive it?
Once your eligibility has been verified you will be able to pick up your smoking cessation products from your local pharmacy.

Who is eligible?
This benefit is available to our 20 years of age or younger members’ parents that are tobacco dependent and have agreed to participate in a Smoking Cessation Coaching program. This benefit is for Texas Children’s Health Plan STAR, CHIP, or STAR Kids members.

24 Hour Nurse Help Line

You can talk to a nurse about your health concerns 24 hours a day, 7 days a week. Just call Texas Children’s Health Plan’s Nurse Help Line toll-free at 1-800-686-3831. Be ready to give your name, member ID number, and your symptoms. Texas Children’s Health Plan’s Nurse Help Line nurses cannot diagnose problems or recommend specific treatments. They can give you health information or tell you where you should go to get health services. They can also tell you how to take care of yourself at home when you don’t feel well, but don’t need to see a doctor. This is not an emergency line or a substitute for your doctor’s care.

Parents of Texas Children’s Health Plan members looking to claim this benefit will need to call 1-800-686-3831.
